Be advised- Vermont NOW ALLOWS patients to designate a dispensary, AND grow their own cannabis!

If you are a current dispensary patient seeking to cultivate for yourself, (or vice-versa) you simply need to submit this form to the state, along with a $25 processing fee.

This same form is also used to change your current dispensary (or other procurement method), update changes to your personal information or grow location, and to replace your lost/stolen card.
